Concussion Management

I have put together this webpage as part of our way of informing parents, students, and families of concussions. I believe it is important to have this information as accessible as possible for educational purposes. In this webpage you will find information regarding facts of concussion, the district protocols, the Athletic Trainer protocol, and a number of links that you can peruse at your own leisure to educate yourself. I hope this information serves as a means to answer your questions.

Thank You,

Eagle Point Athletics

Primary Care Physician Guidelines

If a student elects to see their Primary Care Physician before the Athletic Trainer following a concussion, the following procedure will take place:

1. Student receives OSAA Concussion - Return to Participation Medical Release Form (see link below)

2. The student's Primary Care Physician fills out the form

3. The student then provides a copy of that "Medical Release" back to Eagle Point High School's Athletic Trainer

4. Athletic Trainer then completes the "Return to Play" Protocol

5. Following the completion of RTP, the student will be released by a Doctor's Note that is to be submitted to the Athletic Director
