Primary Care Physician Guidelines
If a student elects to see their Primary Care Physician before the Athletic Trainer following a concussion, the following procedure will take place:
1. Student receives OSAA Concussion - Return to Participation Medical Release Form (see link below)
2. The student's Primary Care Physician fills out the form
3. The student then provides a copy of that "Medical Release" back to Eagle Point High School's Athletic Trainer
4. Athletic Trainer then completes the "Return to Play" Protocol
5. Following the completion of RTP, the student will be released by a Doctor's Note that is to be submitted to the Athletic Director

MAX'S LAW
Max’s Law (OAR 581-022-0421) requires Oregon school districts to implement concussion management guidelines for student athletes (as of 2010–2011).
JENNA'S LAW
Oregon Senate (SB 721) passed an extension of Max’s Law to provide head injury protections to child athletes who play sports outside of school.
