Eligibility

The following requirements MUST be completed, signed and submitted PRIOR to first day of tryouts:

1. EPHS Athletic Code of Conduct
2. Sports Physical (good for 2 years)
3. Medical Insurance - Copy of Insurance Card
4. Participation Fee ($40)
5. Academic Eligibility (OSAA & EPHS)
6. ImPACT Test

Athletic/Activity Code of Conduct


1. Athletic Participation Contract & Code of Conduct

All student athletes and their Parent/Guardian must complete and return the EPHS Participation Contract.

2. Sports Physicals

All student athletes must pass a Sports Physical. Physicals are good for 2 calendar years.

3. Medical Insurance

All student athletes must have health insurance coverage:

  • Personal coverage through parents' employment
  • Oregon Health Plan
  • Purchase School Insurance

4. Participation Fee

All students must pay a $40.00 participation fee (per athlete per year).  Fee must be paid prior to the first contest.

5. Academic Eligibility

All students must meet both OSAA and EPHS Academic Eligibility guidelines.  See the Code of Conduct for more information.

6. ImPACT Test

All incoming freshman & those that have never taken the Baseline ImPACT test will need to take the test in order to be cleared for sports.  Students may take the test during clearance week.


*** HOMESCHOOL STUDENTS ***

(a)     Minimum Test Score.  A home school student who transfers to a public or private school may establish initial academic eligibility by achieving a minimum score on any adopted achievement test pursuant to ORS 339.035.  The student shall achieve a minimum composite test score that places the student at or above the 23rd percentile based on national norms.

(b)     Test Deadline.  The achievement test must be completed prior to August 15 each school year.

(c)      Work Sample Alternative.  In the absence of achievement test results submitted by the deadline of August 15 each school year, a school district may adopt alternative requirements including but not limited to a requirement that a student submit a portfolio of work samples to a school district committee for review to determine whether a student is eligible to participate in interscholastic activities (MUST BE CLEARED BY THE ATHLETIC DIRECTOR)

(d)  Tests approved for assessment of satisfactory progress by home school students are the two most recent versions of the following tests:

Terra Nova Comprehensive Tests of Basic Skills
Iowa Tests of Basic Skills Metroplitan Achievement Battery
Stanford Achievement Test Battery
